- All pdf formatted forms require Adobe Acrobat Reader version 6 or higher to be viewed or printed.
- The Application form and Roster/Release form can be printed in either format offered but results of printing the html page are unpredictable. If you do not have all fonts used, the Word document may also give unpredicatble results.
- The Game Roster can be printed and filled in manually, or can be typed in.
- Each page has rosters for two games. Typing in the team and player information for one game will automatically fill out the other game as well.
- When completed, print or copy 2 sets of sheets, giving a total of 4 game rosters. These will be verified at check in.
- You will need one of these forms for each game.
