TOURNAMENT CHECK-IN INSTRUCTIONS

1. Player Roster/Release Form

Each team should have two copies. We will keep one copy, the other will be initialed and returned to the coach/manager. Click here for the forms page

2. Player Passes

Player passes will be checked against the Player Roster/Release Form.

3. A Copy of your Current State Roster

We will keep the copy. Add information on any guest players to this copy. Please have the original available in case of discrepancies.

4. Medical Release Forms

Medical Release forms will be checked against the Player Roster/Release Form. There must be a medical release form for each player listed.

5. Permission to Travel Form

Required for all out-of-state teams. If your state has not already sent one to us please have a copy available. Region III teams only need a notification of Travel form.

6. Game Rosters (4)

You will need to have 4 completed game rosters (there are 2 on each sheet). These will be verified and returned at check in. You will need one for each game during the tournament. Click here for the forms page

7. Coach's Packet

You will be handed a Coach's Packet. There is a complete set of schedules in each packet. Make sure to check your schedule for any changes.
